In brief

An azole antifungal used in anti-dandruff shampoos, and one of the actives that absorbed the European market after zinc pyrithione was prohibited in 2022. Formulations containing it control visible flaking effectively, including one that matched a 1% zinc pyrithione product in a split-head comparison. Two qualifications matter more than usual here. No published trial has tested climbazole on its own, so every result attributed to it came from a shampoo containing a second antifungal. And the largest relevant trial, in 620 people, set out to compare the two ways of making a shampoo stronger, adding a second active or improving delivery of one, and found that the zinc pyrithione plus climbazole product was beaten at every weekly timepoint by the single-active formulation with better bioavailability. Read that as evidence about how shampoos are built rather than proof that this molecule fails, and read the dual-active marketing with more suspicion than the shelf invites.

European Union
Lawful cosmetic ingredient, subject to concentration limits

Lawful in the EU under Regulation (EC) No 1223/2009, at maximum concentrations that differ by product type and by the job the ingredient is doing. Commission Regulation (EU) 2019/698 set the current position: as an anti-dandruff active it is permitted only in rinse-off shampoo, at up to 2.0% (Annex III, entry 310); as a preservative it is restricted to four categories under Annex V, entry 32 — rinse-off shampoo at up to 0.5%, and hair lotions, face creams and foot care products at up to 0.2%. That 2019 amendment narrowed preservative use, which had previously been allowed across cosmetic products generally. It was unaffected by Commission Regulation (EU) 2021/1902, which prohibited zinc pyrithione in cosmetics from 1 March 2022, and it is one of the actives that absorbed the European anti-dandruff market as a result.

United States
Used as a cosmetic ingredient

Used in the United States as a conventional cosmetic ingredient. It does not hold the over-the-counter anti-dandruff monograph status that zinc pyrithione holds there, so it appears less often in American anti-dandruff products than in European ones.

Two limitations compound each other. The active has not been tested alone in this literature, so its individual contribution is unresolved. And in the largest of the three trials the zinc pyrithione plus climbazole product was beaten by an optimised single-active formulation at every weekly timepoint.

Marketing claim: a second active makes an anti-dandruff shampoo more effective
This compares formulations rather than molecules, and it does not show that climbazole is inert. It does undercut the specific proposition that two actives are better than one, which is how dual-active shampoos are sold.
The premise was tested directly in a randomised double-blind trial of 620 participants comparing the two strategies against each other: adding a second active, or maximising the bioavailability of one (Schwartz 2013). · The single-active formulation with optimised delivery outperformed the dual-active product containing climbazole on both flaking and epidermal histamine, at all four weekly assessments, and delivered more active to the target region of the follicle.

Attributed to climbazole as a single active
This is not evidence of absence, and the mechanism is a reasonable one. It means the confident single-ingredient claims made for climbazole rest on trials that were never designed to support them.
No trial in the available literature isolates climbazole against a vehicle or a comparator on its own. · Unresolved. Every efficacy result attributed to this ingredient was produced by a formulation containing at least one other antifungal active.

The honest part

Two separate problems sit under this ingredient's reputation. The first is that no published trial has tested it on its own, so every figure quoted for it belongs to a formulation containing something else. The second is sharper: the one large trial that set out to test whether a second active helps at all found that it did not, and that a single well-delivered active did better. Climbazole holds its place in Europe because zinc pyrithione was prohibited, which is a regulatory fact rather than a finding about this molecule.

Not the same as its namesake

Climbazole belongs to the azole class and is regularly discussed as though that made it a cosmetic version of ketoconazole. It is not, and this card's grades do not extend to ketoconazole. Ketoconazole is a medicine, used at 1 to 2% under medical regulation, with its own body of controlled evidence in seborrhoeic dermatitis. Climbazole is a cosmetic ingredient with a different potency profile and a far thinner independent record. Shared chemical class is not shared evidence.

Identity

Climbazole is a synthetic imidazole antifungal, a single defined molecule rather than a mixture, used in cosmetics as an anti-dandruff active and as a preservative. It is common in European hair care and comparatively uncommon in American anti-dandruff products, for regulatory rather than technical reasons.

Development & history

Climbazole spent decades as a secondary option in a category dominated by zinc pyrithione, most often formulated alongside another active rather than as the sole antifungal in a product.

Its European position changed in 2022, when zinc pyrithione was prohibited in EU cosmetics following its classification as toxic for reproduction, category 1B. The formulators who had relied on it needed replacements carrying no equivalent classification, and climbazole and piroctone olamine were the available candidates. Neither won a contest. Both inherited a market.

The comparative literature that now supports climbazole predates that shift and was designed around different questions, principally how to make a zinc pyrithione shampoo perform better.

Mechanism (as proposed)

Dandruff is associated with Malassezia yeasts on the scalp and the flaking that accompanies their proliferation. Climbazole, like other azoles, is proposed to inhibit fungal cytochrome P450 enzymes involved in ergosterol synthesis, degrading the integrity of the fungal cell membrane.

The mechanism is well characterised for the azole class generally. What it does not resolve is how much suppression this particular molecule achieves at cosmetic concentrations in a product that is rinsed off within minutes, which is the question the trial design has consistently left unanswered by pairing it with something else.

As with every active in this category, the proposal is suppression of the organism rather than any change to why a given scalp reacts to it, so flaking returns when treatment stops.
