The Anti-Dandruff Aisle Is Different in Europe. Nobody Tells You Why

Ask the internet which anti-dandruff ingredient works best and you get a ranking. Zinc pyrithione, ketoconazole, selenium sulfide, piroctone olamine, sometimes tea tree, ordered by strength as though they were competing on a single scale.

That ranking has a hole in it, and the hole is geographic. A reader in Manchester and a reader in Chicago follow the same advice into two different shops and come out with two different bottles, because one of the ingredients on that list has been illegal in European cosmetics since 2022. Neither of them is told this. And the reason it was prohibited is not the one you would guess.

The short version. Zinc pyrithione controls flaking well and was reviewed by the EU's own scientific committee, which concluded it was safe for exactly that use. It was prohibited anyway, from 1 March 2022, because a hazard classification triggered a legal test that asks a different question and the industry could not answer it. Europe now runs on piroctone olamine, which performs comparably in the trials that exist, with the caveat that those trials never tested it alone. So the European shelf is not weaker. It is just built on a decision most coverage reports incorrectly.

First, what the flaking actually is

Dandruff is not dryness, which is why moisturising the scalp tends to make people feel virtuous and change nothing.

The flaking is associated with Malassezia, a yeast that lives on almost every adult scalp and feeds on sebum. In some people its presence provokes an inflammatory response, faster cell turnover and the visible shedding of clumped corneocytes. Everyone hosts the organism. Not everyone reacts to it.

That single fact organises the whole aisle. Every active worth discussing is an antifungal, and every one of them suppresses the organism rather than changing whatever it is about a given scalp that reacts to it. This is why dandruff comes back when treatment stops, in the same way weeds come back when you stop weeding. Nothing on the shelf is proposed to be curative, and any product implying otherwise is selling you a promise the category cannot keep.

The decision that split the shelf

Zinc pyrithione had held the category for sixty years. It is the active in the shampoos most people can name.

In 2020 the EU's CLP Regulation was amended, giving the substance a harmonised classification as toxic for reproduction, category 1B. Under the Cosmetics Regulation, a substance with that classification can only stay in cosmetics by exception, and the exception has conditions. One of them is that there must be no suitable alternative available.

An application was made to keep it as an anti-dandruff agent in rinse-off products at up to 1%. The Scientific Committee on Consumer Safety reviewed the safety question and, in March 2020, answered it favourably: safe for that use, at that concentration.

Then the application failed anyway, on the other condition. Suitable alternatives plainly did exist. Commission Regulation (EU) 2021/1902 followed in October 2021, moving zinc pyrithione to the list of prohibited substances, and the ban took effect on 1 March 2022, including for stock already sitting on shelves.

Read that sequence again, because almost every article about it gets the causation backwards. The substance was not found to be dangerous in shampoo. It was found to be classifiable as a reproductive hazard, and once it was, the law stopped asking whether it was safe in use and started asking whether it was necessary. It was not necessary. That is the entire story.

Two things are true at once here, and most coverage picks whichever one suits it. Safe in use, and no longer lawful. The gap between those two words is where this decision lives.

Meanwhile in the United States nothing changed. It remains a recognised over-the-counter anti-dandruff active there, and American labels print it as Pyrithione Zinc — same molecule, words reversed.

What replaced it, and how well that is evidenced

Piroctone olamine inherited the European market essentially overnight, and it is worth being precise about how it won, because it did not.

It won by not carrying a reproductive hazard classification. The comparative research that now gets cited to justify it predates the prohibition by a decade or more and was never designed to settle the question that regulation settled first.

That research is decent, and it says roughly what you would hope. In a double-blind bilateral study, a shampoo with 0.75% piroctone olamine and 2% salicylic acid was compared against a 1% zinc pyrithione shampoo over about four weeks; both were highly effective, with the combination scoring slightly better on severity and affected area. In a later split-head study on moderate to severe dandruff, 0.5% piroctone olamine with 0.45% climbazole matched 1% zinc pyrithione for reducing visible flaking, while depositing more active on the scalp after rinsing and leaving hair easier to comb wet.

Now the caveat, which is structural rather than a footnote. In both studies piroctone olamine was tested inside a combination, while the comparator was zinc pyrithione on its own. The formulations were compared. The molecules were not. How much of that performance belongs to piroctone olamine by itself is a question the published record does not answer.

So the honest position is narrower than the marketing: European formulations containing piroctone olamine work about as well as the zinc pyrithione shampoos they replaced. That is a genuinely reassuring finding, and it is not the same as the two actives having been shown equivalent.

The property that decides more than the percentage

One detail from the second study deserves lifting out, because it contradicts how people shop.

The piroctone olamine formulation used half the concentration of the comparator and still matched it, and the measured difference was in how much active remained on the scalp after rinsing. That is substantivity, and for a product you wash off within two minutes it matters more than the number on the box.

An antifungal that leaves with the water does nothing regardless of its concentration. This is why comparing anti-dandruff shampoos by percentage is close to meaningless, and why the same active performs differently across brands: the formulation determines deposition, and deposition determines whether the active is present long enough to act.

It is also the practical reason the standard advice about contact time is not a myth. Leaving a medicated shampoo on for a few minutes is not folklore; it is compensating for the fact that a rinse-off product has very little time to deliver anything.

One flat tie on the outcome most people actually care about, and the rest of the differences are about where you live and what the label carries.

The verdict, laid bare

There is no European downgrade here, and if you moved from an American shampoo to a European one and felt that something was lost, the evidence does not support that feeling.

What did happen is that a decision made on hazard classification got reported as a decision made on safety, and the difference matters for how you read every similar story. A substance can be safe in the use it was sold for and still fail a legal test built to ask whether it is necessary. Those are separate questions with separate answers, and conflating them produces exactly the anxiety this decision generated.

For choosing a product, the useful advice is unglamorous. The active matters less than whether the formulation keeps it on your scalp, so a shampoo you leave on for a few minutes and use consistently will beat a stronger one used briefly. If flaking is greasy, stubborn and does not respond, that is a conversation with a pharmacist or a doctor about ketoconazole rather than a reason to buy a fifth shampoo. And whatever works will keep working only while you keep using it, because none of these actives changes why your scalp reacts in the first place.

The best anti-dandruff active is the one that is still on your scalp five minutes after you started rinsing.

FAQ

Why can't I buy zinc pyrithione shampoo in Europe any more? It has been prohibited in EU cosmetics since 1 March 2022. The trigger was a harmonised classification as toxic for reproduction, category 1B, which meant it could only remain by exception, and that exception requires showing no suitable alternative exists. Alternatives did exist, so the exception was refused.

Was it banned because it was found unsafe in shampoo? No, and this is the part most coverage gets wrong. The qualification is that piroctone olamine was tested in combination with another active in both studies, while zinc pyrithione was tested alone, so the molecules themselves have not been compared directly.

Should I be worried that I used zinc pyrithione shampoo for years? The classification describes a hazard property of the substance, not a measured risk from washing your hair. The committee that examined the actual exposure concluded it was safe for that use. If you would rather avoid it, every market has effective alternatives.

What about American shampoos, since they still contain it? They are lawful where they are sold, and it is listed there as Pyrithione Zinc. Whether importing something the EU has prohibited is sensible is your call; the practical point is that the European alternatives are not a compromise on performance.

Does dandruff shampoo stop working over time? Flaking returning when you stop or reduce use is the norm rather than a sign of resistance, because these actives suppress the yeast without changing why your scalp reacts to it. Before concluding a product has failed, check that you are leaving it on long enough, since contact time affects how much active is deposited.
