Identity
Urea — also called carbamide — is a small, water-loving molecule (CH4N2O) that occurs naturally in the skin as part of the Natural Moisturizing Factor (NMF), the built-in cocktail that keeps the outer skin layer hydrated. Healthy stratum corneum contains roughly 7% urea, and that amount falls with age, harsh climates, and dry-skin conditions. That is the key to why topical urea works so well: it is not a foreign chemical but a replenishment of something the skin already makes and has lost — a biomimetic quality purely synthetic humectants lack.
Two label cautions belong on the identity line. First, urea's behaviour is set by its percentage (below). Second, imidazolidinyl urea and diazolidinyl urea are formaldehyde-releasing preservatives — entirely different substances that merely share the word "urea"; they are not this molecule and are not covered here.
Development & history
- Urea is one of the longest-used and better-evidenced hydrating ingredients, familiar from both heavy-duty foot and callus creams and gentle facial moisturisers.
- Its "foot-cream" reputation comes from the high end of its concentration range; the same molecule at 5-10% is a quietly excellent facial and body humectant — the reputation misrepresents the low end.
- Reviews (2018, 2021) catalogue its moisturising, emollient, keratolytic, barrier-strengthening, and even antimicrobial-supporting properties, with the strongest, most consistent evidence for its core hydration and barrier roles.
Mechanism (as proposed)
Urea works first as a humectant: it is hygroscopic, pulling in and holding water from the environment and deeper skin layers, and — because it is a native NMF component — it integrates with the skin's own moisture system rather than merely coating it. At low, facial strengths (roughly 2-10%) this hydration and barrier support is essentially all it does, and it is where the cosmetic grade sits: better-hydrated, smoother-, softer-feeling skin, well tolerated even on sensitive skin.
Its second action — the one glycerin and hyaluronic acid do not share — is keratolytic, and it appears only at higher concentrations. "Keratolytic" means loosening the keratin that holds dead surface cells together so they shed, smoothing rough, scaly, or bumpy skin. Practically: ~10-20% adds keratolytic smoothing for rough body skin; ~20-30% is stronger, for very rough or scaly areas; and ~40% is proteolytic, a clinical tool that softens nail plate and severe calluses. The reframing that matters is higher is not better — just different: a 5% facial moisturiser and a 40% nail treatment share a name and almost nothing else.
This entry grades the cosmetic use — the low-strength humectant that hydrates and helps skin feel smoother. The mid- and high-strength keratolytic and proteolytic uses, and the dry-scaly conditions urea is used for clinically (xerosis, atopic dermatitis, psoriasis, ichthyosis; keratosis pilaris on the body; nail and callus care), are body-care or medical contexts — noted here for honesty, but not cosmetic facial claims and not graded as treatment. Match the number to the job, and, on skin that does not need it, over-strength urea simply irritates.
