Are Sulfates Bad for Your Skin? The Cancer Myth vs the Real Problem

Sulfates occupy a strange place in skincare. Whole product lines are built around not containing them, "sulfate-free" is a selling point on shampoos and cleansers, and a lot of people avoid them on instinct without knowing what they are or what they supposedly do. The instinct isn't entirely baseless — but it's pointed at the wrong crime.

Sulfates, in cosmetics, almost always mean two ingredients: sodium lauryl sulfate (SLS) and sodium laureth sulfate (SLES). They're surfactants — detergents that lower the surface tension between water and oil, letting them mix so that grime rinses away. They also produce the foamy lather we associate with "clean." They are, in short, very good at cleaning.

The popular fear — that sulfates cause cancer — has no evidence behind it. The real issue is far more ordinary: they're powerful detergents that can strip and irritate the skin they clean. Those are completely different problems, and only one is true.

The cancer myth, and where it came from

There's no credible evidence linking SLS or SLES to cancer. The International Agency for Research on Cancer has not classified either as a carcinogen, and the scientific consensus is that they don't cause it. The myth appears to be a garbled version of a genuine but separate concern.

That concern is 1,4-dioxane — a trace byproduct that can form during ethoxylation, the process used to make SLES (it does not apply to SLS, which isn't ethoxylated). 1,4-dioxane is classified as a possible human carcinogen. But it appears in finished products only at trace levels, and manufacturers use purification steps (sometimes called "vacuum stripping") to bring it below about 1 part per million. In regulated products, the levels aren't considered a meaningful risk.

Here honesty cuts both ways: a precautionary minority points out that purification isn't universally mandated or independently verified in every supply chain, and argues for avoiding ethoxylated ingredients on a "better safe than sorry" basis. That's a defensible preference — but it's a manufacturing-quality concern about one byproduct, not evidence that sulfates themselves are toxic.

The real problem: stripping and irritation

This is the part with actual evidence behind it. SLS is such a reliable skin irritant that dermatology researchers use it as the benchmark irritant in patch testing — the positive control they apply specifically to disrupt the skin barrier on purpose. That's a striking fact: the thing in your cleanser is the same thing scientists reach for when they want to provoke a reaction.

The mechanism is simple. Surfactants don't discriminate well between the oils you want gone and the sebum your skin needs. Strip too much, too often, and you get tightness, dryness, flaking, and a compromised barrier. For most people with healthy skin, using a rinse-off product, this is tolerable — the contact time is short and the skin recovers. But for sensitive, dry, or barrier-compromised skin, it's a problem. The American Academy of Allergy, Asthma & Immunology specifically flags SLS as an aggravator of eczema and recommends non-soap, fragrance-free cleansers for those affected.

SLES is the gentler of the two — ethoxylation makes the molecule larger and less stripping, while it still foams — which is why most modern "mild" cleansers use SLES over SLS. Regulators (FDA, the EU's SCCS, the CIR) consider both safe in rinse-off products at standard levels; irritation risk rises with higher concentration and with leave-on use.

The "sulfate-free" trap

If parabens taught us anything, it's that removing an ingredient isn't the same as improving a formula — and sulfates repeat the lesson. "Sulfate-free" doesn't guarantee gentleness. The replacement surfactants can be equally or more irritating for some people (cocamidopropyl betaine, a common substitute, is itself a recognised contact allergen). Whether a cleanser is gentle depends on the entire formulation — buffering, humectants like glycerin, contact time, dilution — not on the absence of one word.

So should you avoid them?

If you have sensitive, dry, eczema-prone, or reactive skin — or your skin feels tight and stripped after cleansing — a well-formulated sulfate-free (or low-sulfate) cleanser is a reasonable, evidence-aligned choice. If your skin is healthy and a foaming sulfate cleanser leaves it comfortable, there's no toxicological reason to switch. The word on the label matters less than how your skin feels afterwards.
